Treatment of cervical spondylosis can begin with the acute painful phase and progress to the rehabilitative phase (to delay relapse) and preventive phase (to delay the progress of disease process). The cervical spondylosis follows cyclic remissions and exacerbations.
Cervical Spondylosis Treaments
Acute Phase
In this the soft tissues are inflamed and the patient has severe pain. This may last for 3-7 days depending on severity. The patient usually comes to see the doctor when the pain is unbearable and is interfering with his day today activities and it may be following a history of trauma or after repeated episodes of exacerbation and remissions.
The aim of physician in this phase is to relieve pain, soft tissue inflammation and assist in natural resolution of the symptoms. The treatment followed is analgesics for pain and inflammation control, hot water fomentation to relieve the stiffness thus aiding in pain relief and mobility, anti emetics if there is giddiness. Rest should be given to neck during this phase with the aid of a neck collar. Neck traction is avoided during this acute inflammatory phase.
Rehabilitative Phase
As the pain decreases aim is to strengthen neck muscles, increase mobility of neck, relieve pressure on compressed nerves, help the patient correct posture and life style modifications to prevent relapse.?The therapist may use several pain relieving modalities like hot water fomentation, therapeutic ultra sound, trans-cutaneous electric nerve stimulation as needed. Neck traction may be given to relieve nerve compression.
A well trained specialist at give some nerve mobilization and manual mobilization of vertebrae for pain relief, decreasing the compressing force on nerve and increasing mobility.Muscle strengthening of neck and shoulder muscles may be done by the therapist.?Home exercise program which are simple may be given to the patient for continuation during and after the therapy. These may include simple active and passive stretching exercises for increased mobility and isometric contraction exercises for strengthening.
The patient should be instructed to wear a firm (neither hard nor soft) neck collar for at least 3 weeks and should be instructed to use a thin pillow while sleeping or to use a small roll of cloth below the neck so as it supports the curve of the neck. In some cases where spinal cord is compressed, surgery is necessary.
